POLST.org provides information about medical orders that specify the care a seriously ill patient does and does not want to receive.
National POLST, which is based at the Louisiana Health Care Quality Forum (LHCQF), a not-for-profit organization, publishes the website and prepares its content.
Information consists of a section for Patients (including About POLST with background information and links to State Programs); a section for Professionals; charts for patients and for professionals explaining the difference between POLST and advance directives; and Advance Care Planning (explaining how POLST fits in with other types of planning for care during a medical emergency).
